With the goal of providing the best care for Covid-19 patients, minimizing the number of severe cases and deaths, Binh Duong is striving to care for and treat Covid-19 patients. Namely, the province stratify treatment according to the "three layers" model in order to reduce the load on provincial treatment facilities and facilitate the coordination of F0s between layers.
To develop connections of Tele-Health system
The Tele-Health system is a nationwide medical examination and treatment technological system that connects to 100% the district level, allows leading experts to consult Covid-19 patients. According to medical experts, this is one of the effective solutions to support timely the diagnosis and treatment of Covid-19 patients and reduce contact between patients and doctors; helps medical staff at lower levels to feel more secure and confident in treating serious patients.
Referring to the application of the Tele-Health system in the treatment of Covid-19 patients in the province, Dr. Nguyen Hong Chuong, Director of provincial Department of Health, said that in order to improve the capacity of emergency, active resuscitation, response to the collection and treatment of Covid-19 patients, Department of Health directed medical examination and treatment facilities in the province to urgently establish Tele-Health connection between departments, wards within a hospital and between hospitals.
Department of Health assigned Provincial General Hospital to instruct the classification of patient's medical conditions, give emergency advice, actively resuscitate, treat and transfer patients appropriately and promptly to medical examination and treatment facilities in the province by the most convenient means of communication, such as: Viettel, VNPT, Zoom, Zalo, Viber, mobile phone... Covid-19 treatment and sample collection facilities need to arrange human resources, information technology equipment, transmission lines, etc., to immediately connect with upper-level hospitals for periodic and unscheduled remote guidance and consultation.
In recent days, thanks to effectiveness of Tele-Health system in patient care and treatment, more and more patients have been discharged from hospitals. On average, 300 to more than 500 patients are discharged from hospitals every day. Specifically, on August 8, the whole province had 494 patients discharged, bringing the total number of patients discharged during the 4th wave to 5,663. Regarding the transfer of treatment layers, during the day, 11 patients moved from the 1st layer to the 2nd layer, and 22 patients moved from the 2nd layer to the 3rd layer; the number of deceased patients dropped.
To take advantage of golden hours to save people's lives
During a recent meeting on the implementation of measures to prevent and control coronavirus, Provincial Party Standing Committee asked localities to prepare the highest possible treatment options in order to reduce the rate of critically ill patients, take advantage of the golden hour frame to save people's lives. In particular, Provincial Party paid attention to distinguishing infected people without symptoms from those with symptoms to organize appropriate management, monitoring and medical assistance, reducing the rate of asymptomatic patients becoming symptomatic; take the initiative in human resources, facilities and medical equipment, including mobilizing medical resources from other sectors and private sectors for treatment.
Referring to difficulties and challenges in the treatment of F0 cases in Binh Duong, Tran Van Thuan, Deputy Minister of Health, said that the Delta variant has characteristics of rapid spread and rapid clinical development, so mortality rate is higher than previous ones. Patients with rapid clinical developments not only have underlying diseases and are elderly, but also are young people. Binh Duong has actively prepared for monitoring F0 patients when they get worse, but with the current total number of infected cases, treatment facilities are overloaded.
The implementation of home isolation for F0s also concerns provincial leaders, but some cases require medical supervision, so medical equipment (oxygen system, compressed air system, ECMO system) is in great demand. The Ministry of Health immediately sent expert teams from leading hospitals and central hospitals to support Binh Duong. The Ministry of Health also set up a resuscitation center that is located in the province to cooperate with the province's resuscitation centers to receive severe and critical patients. It is expected that when the center is completed, it would reduce the rate of critical patients and mortality.
With the support of the Ministry of Health, the province has proactively set up quick response teams to prepare ambulances, doctors and nurses to approach patients when there is requirement for examination and care. The province's 1022 hotline is also available to receive calls from home treatment cases. In particular, the province has actively built a "three-layer" treatment model according to the patient's clinical progress and level. For the group of mild and asymptomatic patients, the province set up facilities for initial treatment and sample collection, such as taking advantage of dormitories, resettlement areas, gymnasiums, accommodation facilities, hotels...
For the group of moderate and symptomatic patients, the province built field hospitals or set up hospitals to treat Covid-19 patients at district hospitals or private hospitals in the area. Depending on actual conditions, each facility sets up an emergency resuscitation area to treat serious patients. For the group of severe and critically ill patients, the province arranges and ensures the number of ICU beds at provincial and city general hospitals with sufficient conditions for advanced critical care techniques, such as invasive mechanical ventilation, filtering blood for emergency and treatment of severe and critical Covid-19 patients.
